For families with grades K-12 students, the following forms must be completed: - Registration form (K-12 students only)
Attachments required: Proof of age, custody documents/IEP/504 plan as applicable - Home Language Questionnaire (K-12 students only)
- Medical Documentation (all students)
Attachments required: Proof of physical and immunization record - Residency Questionnaire (all students)
Attachments required: Proof of residency (NYS driver’s license, lease, mortgage statement,utility bill, etc.)
For families with universal pre-k students, the following forms must be completed: - Registration form (UPK only)
Attachments required: Proof of age, custody documents/IEP/504 plan as applicable - Emergent Multilingual Learner Form (UPK only)
- Medical Documentation (all students)
Attachments required: Proof of physical and immunization record - Residency Questionnaire (all students)
Attachments required: Proof of residency (lease, mortgage statement, utility bill, etc.)
Please note, in order to submit your online registration, you must click the "save" button on the last page before ending your session. If you see a screen with a session key number, you can be assured that your registration has been submitted.
If you need to submit a new registration, you must start the registration process from the beginning. Session keys cannot be reused.
